Every three years the health department, local health systems, and community partners collaboratively engage to conduct a Community Health Assessment (CHA). The main purpose of a CHA is to complete a comprehensive collection and analysis of data to identify health issues of primary concern. The CHA serves as the basis for the Community Health Improvement Plan as it guides the development of strategies to address the identified priority health issues.
Two pieces of the CHA include sharing preliminary findings with the community at large and actively engaging community members in identifying community health issues they feel are important to address. Reviewing the key findings from the survey and providing your input is encouraged.
Preliminary Key Findings
The preliminary findings of the Community Health Assessment are based on a Community Health Survey that was completed in 2017. Key findings from the survey are available here. The complete Community Health Survey Report is available here.
Primary objectives of the Community Health Survey are to:
- Gather specific data on behavioral and lifestyle habits of the adult population. Select information will also be collected about the respondent’s household.
- Gather data on the prevalence of risk factors and disease conditions existing within the adult population.
- Compare, where appropriate, health data of residents to previous health studies.
- Compare, where appropriate and available, health data of residents to state and national measurements along with Healthy People 2020 goals.

Hard copies of the Priority Health Issues Survey are available at the Fond du Lac County Health Department and public libraries in Fond du Lac County: Brandon, Campbellsport, Fond du Lac, North Fond du Lac, Oakfield, and Ripon.
Last day to provide public input is March 23, 2018 at 4:30pm.
The priority health issues that are selected by the Healthy Fond du Lac County 2020 Steering Committee will be revealed at a community forum in June. More information to come.
