In October of 2014 the Fond du Lac County Health Department convened a group of health care and community partners (task force) to assist with fulfilling their statutory responsibility of completing a community health assessment. This effort also supports the requirement of the Patient Protection and Affordable Care Act (PPACA) for hospitals to do a community health assessment every three years. In additions with United Way’s community impact goal for Building a Healthy Community.
The purpose of this Community Health Assessment is to provide Fond du Lac County with information for an assessment of the health status of residents. Primary objectives are to:
- Gather specific data on behavioral and lifestyle habits of the adult population. Select information will also be collected about the respondent’s household.
- Gather data on the prevalence of risk factors and disease conditions existing within the adult population.
- Compare, where appropriate, health data of residents to previous health studies.
- Compare, where appropriate and available, health data of residents to state and national measurements along with Healthy People 2020 goals.
