In October of 2014 the Fond du Lac County Health Department convened a group of health care and community partners to help gather information on the health of the people living in Fond du Lac County, as required by state statute. This effort also supports the requirement of the Patient Protection and Affordable Care Act (PPACA) for hospitals to do a community health assessment every three years, as well as the United Way’s community impact goal of Building a Healthy Community.
Primary objectives are to:
- Gather specific data on behavioral and lifestyle habits of the adult population. Select information will also be collected about the respondent’s household.
- Gather data on the prevalence of risk factors and disease conditions existing within the adult population.
- Compare, where appropriate, health data of residents to previous health studies.
- Compare, where appropriate and available, health data of residents to state and national measurements along with Healthy People 2020 goals.
This report was commissioned by Agnesian HealthCare, Fond du Lac United Way, Fond du Lac County Health Department, Fond du Lac Family YMCA, and Fond du Lac School District.
