The Family Enrichment Team is a specialized service team within the Department of Social Services which provides a continuum of services to children and families which revolve primarily around Parenting Programs, Home Visitation, In-Home Therapy and related community services.
The Family Enrichment Team's parenting programs are operated under the title of Fond du Lac County Family Foundations POCAN program.
POCAN is an acronym for Promoting Opportunities to Care and Nurture. This program is provided in collaboration with the Public Health Department. POCAN services are designed to strengthen families, enhance child and family development, promote positive parenting practices and link families with appropriate resources and community services. Overall program goals are to provide families with the resources, tools and strategies they need to confront day to day stressors and barriers which contribute to negative outcomes for children. The program utilizes nationally certified parenting materials and assessment tools which are designed to identify areas of need and to promote the development of strategies to address acknowledged family support or family resource needs.
The Family Enrichment Team's component is a wraparound model which utilizes in home services provided by licensed Social Workers. Families may request services directly or may be referred by other community service providers. Families may not be Court ordered into these services, as services are voluntary. Families must be willing to participate in an assessment process and ultimately in the development of an individual family support plan which is designed by the family and identifies the goals and coordination of services that will be individually tailored for the family. There is no cost for the program itself.
